We are able to provide Covid-19  vaccinations for our patient.

Book an Appointment

  • Ring 09 570 2999 or book online using Health 365 (if you do not have access please ring reception 09 5702999 or email info@mtwellingtondocs.co.nz to request access) to see a doctor or nurse.

  • Appointments can be via phone, video call or in-person at the clinic.

  • Please ring 09 5702999 to speak to reception if you are unwell with respiratory symptoms and would like to book an in-person consultation at the clinic. 

  • We are screening all patients for symptoms or contact prior to seeing them.

  • Any patients with symptoms are seen at a different part of the practice. 

Notes:

  • If your regular doctor is away or unavailable, you can see any of our doctors. If you are not sure who you should see, our nurses will be happy to advise you.

  • Standard appointments are for 15 minutes including clinical note writing and referrals. If you feel that you may need longer or have more to discuss, then please book a double appointment. There is an additional charge for longer appointments.

  • If this is your first time visiting us, you will need to book a 15-minute appointment with the nurse & doctor. This is so we can collect all the necessary information and get an idea about your medical background.

 

You can also download & fill an enrolment form and email this back to us in PDF format, with your ID (passport/visa). Email: info@mtwellingtondocs.co.nz

Please bring with you any relevant medical documents that you may have (for example, recent hospital discharge summary) and a list of all medications that you take.
